Dance classes can involve risk of personal injury. While The “Holiya Dance Academy” takes all reasonable care in the conduct of its classes, it accepts no responsibility for injury or loss caused during classes or whilst participants are at or near the dance studio. You are responsible for ensuring that you are physically and medically fit for the class and during the class you must at all times take care of your own personal safety. I promise to “Holiya Dance Academy” that all information on this form is correct. I acknowledge that I will not have any claim of any kind or nature against the “Holiya Dance Academy”for any illness, injury or adverse change in medical condition or state of health affecting myself and/or my child, arising directly or indirectly from any test, training, course or program my child undertakes at “Holiya Dance Academy”. Any rights granted to me by law which are not capable of change by agreement remain unaffected by the terms of this agreement.

I acknowledge that refunds are not available or applicable in regards to my and/or my child's dance tuition if classes are missed and/or participation stops. I further acknowledge that “Holiya Dance Academy” will offer amicable options and alternatives if my and/or my child misses dance classes and accept that this does not include refunding missed lessons or payment on a pro-rata basis. I have explained to my child/ren the behaviour that is expected of them to ensure their safety at “Holiya Dance Academy”.

I take full responsibility for my child's conduct and actions whilst at “Holiya Dance Academy”. I acknowledge that The “Holiya Dance Academy” has disclosed to me that I and my child/ren may be photographed, videoed, filmed or otherwise recorded whilst participating in The “Holiya Dance Academy”Concert and activities and consent to this. By signing this Registration Form I also consent to “Holiya Dance Academy”using my and my child/ren image/s in any promotion, advertisement or other material or activities in relation to “Holiya Dance Academy”and agree that “ Holiya Dance Academy”is entitled to entire copyright in and sole ownership of such images. The Holiya Dance Academy encourages you to utilise social networking knowledgeably, accurately, and carefully.

Your web interactions can result in the public forming opinions about our school, its employees, students and other members. Whether you are blogging, posting, or uploading, in personal or professional social networking it is always important that the context and content be appropriate as it relates to Holiya Dance Academy , and that the values we teach our children are exemplified in our own personal use of social networking. Under no circumstance should offensive comments be made about students or colleagues. Do not comment on or forward unsupported information, e.g. rumours.
